Your satisfaction is our priority. If you are not fully happy with your purchase, you may request a refund within 60 days of delivery under the conditions outlined below.

1. 60-Day Money-Back Guarantee

Forma Dose offers a 60-Day Money-Back Guarantee on eligible products where such a guarantee is expressly offered at the time of purchase.

If you are not satisfied with your purchase, you may request a refund within 60 days of confirmed delivery, subject to the terms and limitations outlined in this Policy.

2. Eligibility Requirements

To be eligible for a refund, all of the following conditions must be met:

• The refund request must be submitted within 60 days of the delivery date
• The request must be submitted by the original purchaser
• The product must have been used only as directed and for personal use
• The request must comply with all terms of this Policy

Refund requests submitted after the 60-day period are not eligible and will not be approved under any circumstances.

3. Product Returns

Due to the nature of consumable products:

• Physical returns are not required and will not be accepted for standard refund requests
• Customers should not mail products back unless explicitly instructed by the Company

4. Refund Limitations & Exclusions

The following are not eligible for refunds:

• Refund requests submitted outside the 60-day guarantee period
• Original shipping and handling fees
• Import duties or taxes
• Requests that do not meet the eligibility requirements stated in this Policy
• Multiple or excessive refund requests from the same customer, household, billing profile, or payment method (at the Company's discretion)

No exceptions are made beyond the 60-day money-back guarantee.

5. Damaged, Defective or Incorrect Items

Please check your order carefully upon arrival. If you have received a damaged, defective, or incorrect item, contact us straight away at support@formadose.shop so we can resolve the issue promptly.

• All return shipping costs will be covered by us
• We will arrange a replacement or full refund at your preference
• Please do not return any items without contacting us first

6. Exchanges

To exchange an item, please first contact us at support@formadose.shop to initiate the refund request process. Once we have received and approved your return, we will arrange your exchange and dispatch the replacement. Return shipping costs for exchanges remain the responsibility of the buyer.

8. Refund Processing

After we receive and review your request, we will notify you of our decision. Approved refunds are issued to the original payment method used at checkout, typically within 5 business days of approval. Please allow additional time for your bank or card provider to process and reflect the payment.

If 10 or more business days have passed since your refund was approved and you have not yet received it, please contact us at support@formadose.shop and we will investigate.

9. Customs, Duties and Taxes

For orders shipped to the USA: No additional fees apply unless the order value exceeds $600.

Original shipping fees and any import duties or taxes are non-refundable in the event of a standard return.

10. Fraud, Abuse & Policy Enforcement

The Company reserves the right to deny refund requests in cases involving:

• Suspected fraud or abuse
• Excessive or repeated refund requests
• Chargeback abuse
• Violation of product use instructions
• Attempts to circumvent this Policy

We may also restrict or refuse future purchases at our discretion. Chargebacks filed for issues covered under this Policy may be disputed and may result in account termination.
